Thinking about each patient in particular and their environment is what allows us to think outside the box

DR. CARLOS GIORDANA

Head of the Cardiology Division of Hospital Misericordia Nuevo Siglo de Córdoba (Argentina)

The Argentine cardiologist from Córdoba, Dr. Carlos Giordana, is a tenacious user of the Practicum Script clinical simulation platform. At the age of 42, he is the Head of the Cardiology Division of the Hospital Misericordia Nuevo Siglo, a position he combines with his practice at the Reina Fabiola University Clinic. He is also part of the teaching staff at the National University of Córdoba as an assistant professor. The 30 cases "have helped me experience real clinical cases of a wide variety of situations, offering an online opportunity for learning and deciding which approaches to take, comparing these approaches with those chosen by experts."

March 21, 2019. In medical practice, we know a lot about statistics and numbers but sometimes, nothing about the gaps between so much data. And yet, "no two patients are the same." This is the opinion of Dr. Carlos Giordana, an Argentine cardiologist resident in Córdoba: "thinking about each patient in particular and their environment is what allows us to think outside the box." According to his perspective, "for each particular case, we should look for unique answers instead of dealing with pre-packaged solutions that can be useful in some situations, but may lead us to an endless number of important errors."

Being able to train clinical reasoning and exercise thinking skills in controlled simulation environments "is vitally important, since they allow those who use these (features) to make committed clinical decisions and know their implications without putting real patients at risk." Practicum Script, in particular, has provided specialists with a complete review of all cardiology subspecialties. According to Dr. Giordana’s words: "It allows us to experience real clinical cases of a wide variety of situations, with their subsequent progressions, offering an online opportunity for learning and deciding which approaches to take, comparing these approaches with those chosen by experts."

His biggest challenge during the course? "The cases involving arrhythmias: I think they are all challenging for any clinical cardiologist like me," says Dr. Giordana. The doctor is in charge of the Cardiology Division of Hospital Misericordia Nuevo Siglo, a public acute care hospital in Córdoba, Argentina. He is also a professor of Internal Medicine at the National University of Córdoba, and his main interest in cardiology is imaging, in which he is fascinated by echocardiography and stress echocardiography.

Dr. Giordana has completed the Second Practicum Script Advanced Cardiology Simulation Course through an agreement that Practicum maintains with the Argentine Federation of Cardiology (FAC). Accumulating a total of 8 years in teaching, he considers the academic results particularly valuable. Compared with the panel of experts, his indicators include coincidence rates of 70% in “hypotheses” and 85% in “scenarios.” His first participation in the forum involved compatibility of diagnostic hypotheses. After being approved, he requested his academic certification as an Expert in Clinical Reasoning, provided by the Distance University of Madrid.

Testimonies

For me, the program is very interesting, since it allows me to sharpen my interrogation and direct it more towards a specific pathology and to have more knowledge of laboratory and cabinet exams. I think it is very important that we keep our medical sagacity throughout our practice and that in the few minutes it takes to answer any of the cases that Practicum Script presents, you acquire more knowledge.

I would like to express my gratitude to the Practicum Institute of Health Sciences Research for the opportunity given to Latin American physicians  to access and take different courses of our specialty for the benefit of our dear patients, in my case in Mexico. It has encouraged me to improve myself and keep abreast of the advances in Pediatrics and put into practice the knowledge acquired in the hospital where I work. Thanks friends and congratulations to this valuable team of Practicum Script Education.

I am grateful for this study tool that, through Nestlé and with this method, allows us to review different issues with cases that confront us in decision-making and serve us in the daily consultation.

Dr. Josué Refugio de León Padilla

Pediatrician in Jalisco (Mexico)

It is an excellent educational support platform for the pediatrician, offers very specific clinical challenges with alternatives for diagnosis and differential diagnoses, with the support of experts, a bibliography and a forum to share opinions. It's easy to navigate and it's fun, a tool that's worth using and making the most of it.
